This study sought to systematically explore the current functioning and sustainability of Aboriginal health workers (AHWs) in the primary healthcare system (PHC) of New South Wales (NSW), Australia. To do so, a literature review was conducted, and supplemented with direct engagements with AHWs and their supervisors at participating services, with conversations guided by the indigenous storytelling approach to knowledge generation, called Yarning, in this context. The resulting yarns (i.e., conversations) highlighted five categories of change that are required to ensure AHW sustainability: community connection, recognition, value, support, and an inclusive health system. This evidence suggests that for AHW roles to be sustainable, a system-wide paradigm shift is needed to better incorporate Aboriginal approaches and knowledge into the PHC system.
