Utilizing Perinatal Patient Navigation to Address Self-Identified Social Determinants of Health in Pregnancy

Maternal health issues, particularly among non-Hispanic Black women, remain a challenge in the U.S. despite medical advancements. Disparities stem from various factors, including community, hospital, provider, and patient influences. Community-based care navigation, using patient navigators and community health workers, has been effective in improving maternal healthcare by increasing access and engagement. This study looks at the social needs of patients referred to a new perinatal patient navigator program in a county with high social vulnerability, aiming to guide best practices for addressing health disparities.
