Ethiopia’s Health Extension Program (HEP) aims to increase primary health care access through the work of CHWs, known as Health extension workers (HEW). This study aimed to determine the top factors of attrition among HEWs. Through a mixed methods approach, the study determined that factors associated with psychosocial issues, administrative constraints, incentives, lack of career pathways, and working environment were causes of high attrition. The study suggests that policies that govern HEW’s should be critically examined to increase their job satisfaction and career growth.
