This study focused on helping Spanish-speaking trauma and burn patients navigate their care after hospital discharge. The program provided resources and support through community health workers, who connected patients to needed services like food, transportation, and mental health care. Of the 80 patients who participated, many had unmet needs such as food and mental health support. The program successfully connected patients to resources, improving access to care, especially in mental health. It highlighted the unique challenges these patients face and the importance of community support in their recovery.
