People released from jail often face serious health challenges, especially those with HIV and substance use issues. A pilot program using Community Health Workers (CHWs) provided support by connecting participants to resources through phone calls and in-person meetings, focusing on substance use, housing, and health. Participants who received more support showed better HIV control, reduced stimulant use, and improved attendance at medical appointments. The CHWs’ empathetic and motivational approach helped participants stay engaged and accountable. While the program showed promise in improving outcomes, it requires significant resources, and future efforts should focus on making it more effective.
