This study evaluates the impact of the 2018 state certification for community health workers (CHWs) in Massachusetts. Surveys were conducted with CHWs and their employers in 2016 and 2021 to assess the effects of certification. The results showed that certification led to higher pay, better perceptions of CHWs, and better integration into care teams for certified CHWs. There were no negative changes in the workforce or employer characteristics after certification. Certified CHWs were more likely to work in large, clinical organizations, while uncertified CHWs often worked in medium-sized community-based organizations. Overall, certification was associated with positive outcomes and did not create workforce disparities. This research can guide future CHW certification and program efforts.
