Testing an Innovative Approach to Improve Hypertension Management at a Federally Qualified Health Center

Hypertension control rates are still inadequate in the U.S. and Los Angeles County. To address this, QueensCare Health Centers launched a team-based hypertension management program led by clinical pharmacists. The program tackled barriers at the system, provider, and patient levels. Strategies included integrating self-monitored blood pressure data into electronic health records, retraining clinicians, adding a community health worker to the team, and using pharmacists to manage medications. Patient-focused efforts involved simplifying educational materials, offering personalized education and support, and providing blood pressure cuffs and pedometers. This intervention demonstrates how team-based care can be effectively implemented at a Federally Qualified Health Center.
