This study found that a 12-month program combining community health workers, telehealth, and regular communication with healthcare providers helped low-income adults with type 2 diabetes better manage their condition. Participants had better blood sugar and cholesterol levels, received more recommended diabetes checkups, and got help with issues such as medication refills and access to care. The findings suggest that better coordination between CHWs and healthcare teams can improve diabetes care in underserved communities.
