The COVID-19 pandemic has renewed focus on community-led health efforts, with states increasingly partnering with community health workers (CHWs) to support underserved communities. Evidence shows CHWs improve access to care and health outcomes while reducing disparities, driving state policies to strengthen and fund their roles. NASHP’s Community Health Worker Learning Collaborative helps states share best practices, with a focus on sustainable funding through Medicaid and other sources. Many states are using Medicaid State Plan Amendments, waivers, and managed care contracts to fund CHW services, each with its own opportunities and challenges.
