The objectives of this study were to examine how different stakeholders define sustainability and to identify barriers to and facilitators of the sustainability of the Afghan CHW program. 63 key informants were interviewed, and 11 focus groups were conducted with policymakers, health managers, community health workers, and community members. The interviews and focus groups showed that stakeholders define sustainability differently. Policymakers emphasized financial resources; health managers emphasized organizational operations; and community-level stakeholders emphasized routine frontline activities. Facilitators identified include integration into the health system, community support, and capable human resources. Barriers included lack of financial resources, poor program design and implementation and poor quality of services. Authors conclude that sustainability is complicated; stakeholders should be addressed in the program design along with a coordinated approach.
