The first 28 days of life are critical for newborns, with millions of deaths occurring globally, especially in sub-Saharan Africa. Efforts to reduce neonatal mortality focus on improving care continuity after discharge from healthcare facilities. This review examined interventions in low- and middle-income countries (LMICs) aimed at enhancing post-discharge care for newborns. Most interventions involved community health worker-led education during home visits, emphasizing improved information flow. However, there is a need to address high-risk newborns and incorporate interpersonal continuity, recognizing the importance of relationships within families and communities for successful interventions. Further research should focus on high-risk neonates to improve outcomes in LMIC settings.
