Reimagining Primary Health Care: A Historical and Contemporary Review of Community-Based Care Models and Innovations

Case studies from countries like Nepal, Ethiopia, Brazil, and Rwanda showed clear health gains. Future challenges include urbanization, changing demographics, and complex health crises. Digital tools can help but need careful, equitable use. Sustaining community-based care will require strong governance, workforce support, technology, and active community involvement.
