The goal of this study was to examine whether a complex intervention implemented by the Government of Ethiopia was associated with improved management of sick children by health extension workers. This complex intervention entailed community engagement, capacity building of health workers, and enhanced district-level ownership of sick child management. The study was conducted in four Ethiopian regions, and data was collected via surveys and consultation observations. The results demonstrated that the intervention was not associated with improved management of sick children by health extension workers’ management, suggesting that training healthcare providers without ensuring continued clinical mentoring and support has limited effect on quality of care. These findings emphasize the need for strengthening community-based programs by ensuring high coverage and continued clinical mentorships, supportive supervision, and supply of medicines and other essential commodities.
