This study evaluated a hypertension (HTN) health education program in Puerto Rico, led by community health workers (CHWs) using the American Heart Association’s Empowered to Serve modules. CHWs taught participants about HTN prevention and management, provided blood pressure monitors, and referred them to self-monitoring programs. Pre- and post-surveys showed significant improvements in participants’ confidence in managing blood pressure, making lifestyle changes, reading nutrition labels, and reducing processed foods. These results highlight CHWs’ role in improving HTN knowledge and promoting healthier behaviors in communities.
