In Singapore, the “Get Well, Live Well” program helped older adults improve their health knowledge, adopt healthier habits, and feel more socially connected. Community Health Workers (CHWs), called Care Connectors, played a key role by offering personalized support, building trust, and guiding participants effectively. The program’s success highlighted the importance of CHWs with strong interpersonal skills for future community health initiatives.
