Chronic Kidney Disease (CKD) affects many adults in the U.S., especially in underserved Latine communities where awareness and screening are limited. This study evaluated a program led by Community Health Workers (CHWs) to improve CKD knowledge and care among Latines with low health literacy. Using a culturally tailored CKD Flipchart, 100 patients participated, and 85% reported plans to seek kidney care. Medical records showed improved blood sugar and triglyceride levels after the intervention, suggesting CHW-led programs can help reduce CKD health disparities in underserved communities.
