Patient Perceptions of a Digitally Enabled Community Health Worker Intervention: Qualitative Study Among Pilot Trial Participants

This study found that combining a digital health platform with support from community health workers helped people with heart failure better manage their condition after leaving the hospital. Patients found the technology easy to use, gained more confidence in caring for their health, and valued the emotional support and practical help community health workers provided, such as connecting them to needed services. Although some technical issues occurred, the approach shows promise for improving home-based care for people with heart failure.
