This study explored how people with heart failure experienced using a digital health platform together with support from a community health worker after leaving the hospital. Participants found the digital tools easy to use and said they helped them better understand and manage their condition. Community health workers made the technology easier to use, provided emotional support, helped solve everyday challenges such as transportation and food access, and encouraged patients to follow their care plans. Although some technical issues occurred, the combined approach was well received and showed promise for improving care at home.
