Medicaid Health Homes (MHHs) are an innovative way to engage CHWs, seeking to improve patients’ health by providing care coordination for chronic conditions via virtual health care networks for states with expanded Medicaid access. Through qualitative interviews, this study aimed to understand the perceptions of MHH administrators regarding CHW engagement in MHHs, along with the facilitating factors and barriers in adopting MHHs. The study shows that barriers prevent CHWs from being integrated into all MHHs in some contexts, however there are opportunities for further CHW integration.
