The primary goal of this study was to evaluate the impact of a generalized (i.e., not disease-specific) health plan-led CHW program on overall healthcare use and spending. Retrospective cohort study data was utilized from adult members who received the CHW intervention and compared with data from those who were identified for participation but could not be reached. The outcome measures of interest, evaluated after 6 months follow-up, included healthcare utilization, emergency department visits, and outpatient visits. Subsequent results demonstrated that program participants experienced a greater increase in outpatient evaluation and management visits than the comparison group during the first six months of the program. This successful increase implies that health plans may be particularly well positioned to finance, sustain, and scale programs, including those related to CHWs, that address social drivers of health.
