This study explored what affects starting and sticking with antiretroviral therapy (ART) for HIV in Nepal. Interviews with 24 people living with HIV and 26 healthcare providers revealed several key factors. On the positive side, personal fears of death, knowledge about ART, and financial support helped people start and continue treatment. Challenges included concerns about lifelong medication, side effects, stigma, and financial issues. Family support and community health workers played a big role in helping with treatment, but social stigma and logistical problems, especially during COVID-19, made things harder. To improve ART uptake and retention, efforts should focus on raising awareness, improving healthcare systems, ensuring consistent medication availability, and creating supportive environments at all levels.
