Community health workers help people deal with everyday challenges that affect their health by connecting them to services, offering support, and helping them make healthy choices. Research shows that strong CHW programs improve long-term health, support mental wellbeing, reduce hospital visits, and help more people access basic care. While these programs have often depended on short-term funding, many state Medicaid programs now pay for CHW services. This toolkit offers simple examples to help states and advocates include CHW services in Medicaid plans and payment policies
