This review explored how community health workers (CHWs) and pharmacists collaborate to improve patient care. CHWs collected patient information, helped with self-management, translated information, and ensured follow-ups, while pharmacists focused on medication and patient education. They worked together in various ways, from sharing information to delivering services as a team. Their partnership helps bridge cultural gaps, improve referrals, and address social factors affecting health, leading to better pharmaceutical care and health outcomes.
