This report discusses a partnership between social workers (SWs) and community health workers (CHWs) to address social determinants of health (SDoH) for high-risk patients in an urban safety-net hospital system. CHWs, who share experiences and understanding with the communities they serve, bridge health, social, and community systems. While CHWs have shown effectiveness in various health areas, challenges remain in understanding their integration into health systems and defining their roles. The partnership aims to improve patient outcomes by addressing unmet social needs, with promising early results in reducing readmissions. The program is scalable and may serve as a model for other hospital settings seeking to leverage CHWs to enhance patient care.
