This review examined how integrated care for older adults is being implemented in primary health care around the world. Across 181 studies from 26 countries, most programs used team-based care, coordinated services, and personalized care plans, often combined with health education, physical activity, and psychosocial support. Most studies reported improvements in patient health, showing that coordinated care can help meet the complex needs of older adults. Programs that use existing primary health care services, involve community health workers, and use simple digital tools appear most practical to expand. However, more evidence is needed from low- and middle-income countries to understand how these approaches work in resource-limited settings.
