Implementing a social needs screening and community health worker referral program for obstetric patients

This study looked at a new program that screened pregnant and postpartum patients for social needs like food, housing, and transportation, and connected them to Community Health Workers (CHWs) for help. Out of 3,275 patients screened, 489 had at least one need, and most were referred to a CHW. About two-thirds of those referred were successfully connected to support services, and nearly all of them said their needs improved or were fully resolved. The program shows promise in helping patients get the support they need beyond medical care.
