This study introduces a digital diagnostic and monitoring tool for primary healthcare in São Paulo, implemented in 14 healthcare units. It addresses challenges in territory management faced by healthcare professionals. The tool, developed after analyzing existing instruments, improves data reliability, visualization, and work process efficiency. Results suggest successful integration into daily work with potential for further improvement.The relevance for Community Health Worker (CHW) data management lies in its ability to streamline processes, enhance data accuracy, and support CHWs in effectively managing healthcare territories.
