Implementation fidelity to a behavioral diabetes prevention intervention in two New York City safety net primary care practices

The purpose of this study was to test the impact of a health coaching intervention seeking to prevent Type 2 Diabetes in New York. This intervention, Community Health Outreach to Reduce Diabetes (CHORD),  was a cluster-randomized trial examining a CHW education intervention to reduce Type 2 Diabetes among pre-diabetic patients. The results reported in this publication cover only the assessment of the intervention and not its outcomes. Results indicate that CHWs are able to implement the CHORD intervention but that fidelity varied across two implementation sites.
