Healthcare interventions involving community healthcare workers in the management of people with multiple long-term conditions: scoping review

by

Authors: Olona Mzimkulu, Lara Fairall, Max Bachmann, Tamara Kredo, Linda Velapi, Robyn Curran, Inge Petersen, Khosi Simelane, Daniella Georgeu-Pepper, Naomi Levitt

This review looked at how community health workers support people living with several long-term health conditions at the same time. Fourteen studies showed that CHWs help patients manage their care, provide health education and encouragement, offer coaching and support, and follow up or connect patients to health services. They use tools such as mobile apps, activity records, booklets, and videos to support their work. The study suggests that CHWs can play an important role in helping people manage multiple health conditions and improving coordination of care in communities.
