Somalia’s health system faces major challenges due to weak governance and conflict. To improve services, two new efforts were introduced: the Marwo Caafimaad community health worker program and a digital reporting system using DHIS2. This study reviewed health data from 2018–2024 and gathered insights from interviews and focus groups. The results showed clear progress; antenatal care, skilled birth attendance, and child immunization rates all improved, especially in areas using both community health workers and digital tools. The study concludes that combining local health workers with transparent digital systems can strengthen health services and rebuild public trust in fragile settings like Somalia.
