This article is based on ethnographic research (2019-2020) aimed at examining a new national CHW programme that has been introduced in Zambia. The results of this research uncovered similar problems suffered by the new CHW programmes to those that affected the programmes of the Alma-Ata era. Researchers discuss how these mistakes have been repeated and ask why the lessons of the Alma-Ata era have been lost. Given the central problems identified, researchers recommend that national CHW programmes be better funded and respected, that global health organizations and actors reflect on the historical experiences and qualitative research of the past thirty years, and that policymakers create clear and realistic expectations for CHWs moving forward.
