Financing Thresholds for Sustainability of Community Health Worker Programs for Patients Receiving Medicaid Across the United States

The study examines Medicaid payment rates required to sustain community health worker (CHW) programs across the US. Using a microsimulation model, it estimates costs for CHW salaries, equipment, transportation, and benefits. Findings reveal that current payment rates are insufficient for program sustainability. Higher fee-for-service and capitated rates may be necessary. However, challenges exist in accurately estimating these rates due to variations in program structure and local factors. Clarity and transparency in payment determination are essential for supporting CHW programs effectively.
