Feasibility and acceptability of home-based neonatal hyperbilirubinemia screening by community health workers using transcutaneous bilimeters in Bangladesh

The purpose of this study was to evaluate the acceptability and operational feasibility of a CHW-led, home-based, non-invasive neonatal hyperbilirubinemia screening using a transcutaneous bilimeter in Shakhipur, a rural sub-district in Bangladesh. First, focus group discussions and key informant interviews were conducted with parents, grandparents of infants, and public and private healthcare providers and managers, to explore their current knowledge, perceptions, practices, and challenges. Next, a prenatal sensitization intervention and home-based screening by CHWs was conducted to evaluate the acceptability and operational feasibility of this approach. Results from the formative findings identified misconceptions regarding neonatal hyperbilirubinemia causes and health risks among caregivers. CHWs were comfortable with the adoption, maintenance, and use of the transcutaneous bilimeter device in routine home visits. In addition, transcutaneous bilimeter-based screening was widely accepted by caregivers and family members due to its noninvasive nature and immediate display of findings following at-home administration. These findings suggest this approach may offer an acceptable intervention model to increase rates of neonatal hyperbilirubinemia screening,  while alleviating the work burden of CHWs.
