Heart failure (HF) is a major cause of hospital readmissions, posing challenges for patients. Combining remote monitoring with community health worker (CHW) support can improve HF management. This study aims to assess the acceptability, feasibility, and effectiveness of such an intervention for HF patients transitioning from hospital to home. Adult inpatients were randomized to receive either the intervention (digital platform+CHW support+usual care) or control (CHW support+usual care). Feasibility measures include device usage rates, while acceptability is assessed through patient willingness to use the digital platform. Preliminary effectiveness is tracked via 30-day clinical outcomes. Initial results are expected to inform future trials aimed at enhancing HF care with digital interventions and social support.
