Ensuring fair compensation for community health workers (CHWs) is crucial for their sustained impact on health equity worldwide. This review examines six compensation models using empirical literature and the CFIR framework. It identifies factors influencing CHW effectiveness and their impact on global health equity. Grant-based models show promise but face sustainability challenges, while volunteer models risk CHW burnout. Financial security, motivation, and community trust are crucial for CHW sustainability. Policymakers must consider these factors to support CHWs effectively and ensure equitable health service delivery.
