Experiences from the Philippine grassroots: impact of strengthening primary care systems on health worker satisfaction and intention to stay

 This paper sought to underscore the value of strengthening primary care system interventions in LMICs by examining the impact of system strengthening interventions on job satisfaction and intention to stay among healthcare workers in the Philippines. Implemented interventions included the introduction of performance-based incentives, the adoption of electronic health records, and the enhancement of diagnostic and pharmaceutical capabilities over a 1-year period. A questionnaire examining job satisfaction (motivators) and dissatisfaction factors was distributed before and after intervention implementation to healthcare workers, including family physicians, nurses, midwives, and CHWs. Results revealed a significant improvement in perceived compensation fairness among urban and rural providers and a significant improvement in medicine access for the rural workforce. Job motivation and intention to stay was also sustained in urban and rural settings between periods. While select interventions proved promising in better-supported settings, the immediate impact of these interventions was inadequate in off-setting the infrastructural and staffing gaps experienced in disadvantaged areas. Hence, these findings emphasize a need for these problems to be comprehensively addressed to increase job satisfaction and attrition for healthcare workers, such as CHWs, and to better serve marginalized communities.
