This study explores the effectiveness of a community-based patient navigation program for breast cancer screening. Using a sustainability framework, the program secured funding from both city and state health departments, expanded institutional support, and engaged policymakers to advocate for training and reimbursement for patient navigators and community health workers. The program’s impact extends beyond individual outcomes, influencing community, organizational, and policy-level changes, highlighting its sustainability and broader reach.
