Innovative approaches are needed to provide comprehensive primary care, especially in developing countries where traditional models face challenges. The evolution of the Community Health Worker (CHW) model offers promise. This study examines the comprehensive provider stage of the CHW model using Qualitative Comparative Analysis (QCA). Six programs were analyzed, revealing key characteristics for success. Programs with over 80% of identified characteristics showed superior performance. Foundational elements included close CHW supervision, care coordination, defined referral pathways, medication management, proactive care, and cost-effectiveness. Essential added elements for high performance included full population empanelment/enrollment, comprehensive assessment, risk stratification, defined care protocols, and integration of cultural wisdom for community engagement.
