This study in Harare, Zimbabwe tested whether community health workers could help people with diabetes and high blood pressure better manage their conditions. Six clinics were involved, with over 600 patients split into two groups. One group received education sessions and monthly home visits for six months, while the other received usual care. People who got support from community health workers were more likely to follow healthy habits like eating better, exercising, and reducing alcohol use. They also took their medicines more regularly and had better blood pressure control. However, there was no clear difference in blood sugar control or smoking habits. Overall, the study shows that community health workers can improve how patients manage long-term conditions in low-resource settings.
