Evaluating the Effectiveness of a Patient-Centered, Nonphysician Led Self-Monitoring Blood Pressure Program in a Rural Federally Qualified Health Center

Uncontrolled hypertension increases the risk of serious health issues. This study evaluated a self-monitoring blood pressure (SMBP) program at a rural health center in Washington, focusing on nonphysician involvement. Patients with uncontrolled hypertension used BP cuffs to log daily readings and met with community health workers (CHWs) to set management goals over 3-4 months. The program, serving 205 patients, faced barriers like lack of reimbursement for nonphysician time and BP monitors but benefited from expanded telehealth and adapted resources during COVID-19. While it successfully integrated CHWs and addressed patient needs, there was no significant improvement in blood pressure outcomes. The study highlighted the importance of reimbursement policies for sustaining SMBP programs.
