This study assesses Health Extension Worker’s (HEW) pre-service training status and perceived competency in Ethiopia. The study used a mixed methods approach to investigate Health Extension Program (HEP) activities by sampling 515 HEWs, 1,245 HEW trainees, 192 instructors, and 43 key informants. Results show 26% of HEWs showed competency in all the HEP activities, while 73% of HEWs said they could deliver 75% of the HEP activities. However, while the perceived competence of HEW trainees is high, training is not provided as per the curriculum because of limited resources.
