Community health workers (CHWs), often women without formal health training, play vital roles in addressing health disparities. This study examines a program employing CHWs as “change agents” to provide nutrition education in eastern India. It explores how different forms of voluntarism, such as laissez-faire and honorarium-based, influence CHW efficacy, empowerment, and equity. Results show that while unpaid positions may lead to volunteer selection based on logistical reasons rather than motivation, greater responsibilities can enhance empowerment but may exclude marginalized individuals. Payment can affect motivation and management, with varying perceptions among CHWs. Overall, offering honorariums could enhance inclusivity and effectiveness in CHW programs, especially for marginalized women.
