Enhancing And Sustaining The Memory Care Navigator Model In Hawaii

Hawaii’s memory care navigator model helps local families by connecting them with dementia resources and support. Community Health Workers were trained to provide personalized, culturally appropriate care and use a digital platform to track assessments and care plans. The program is part of Hawaii’s workforce development, with training materials available online for ongoing education and use in community college programs. This model has improved coordination in dementia care across the state.
