Disparities in hypertension control exist across race, ethnicity, and language in the U.S. This study at 15 clinics tested if an intervention could improve hypertension control and reduce these disparities.Primary Care Clinics (PCPs) used an equity dashboard, conducted equity huddles with Population Health Coordinators (PHCs), and received support from Community Health Workers (CHWs). Among 10,865 target patients, the intervention modestly but significantly improved hypertension control by 0.9%, with a 2.1% greater improvement compared to White, English-speaking patients. Using disparity data in clinical practice, along with PHC and CHW support, proved effective.
