The SILOS program helps reduce social isolation among older adults by connecting them to community services and resources. Using tools to assess social disconnectedness and support, healthcare workers and Community Health Workers (CHWs) screened participants and tracked progress over 12 months. The program showed significant improvements in social connectedness and support for participants, most of whom were at risk of isolation. CHWs played a key role in building trust and addressing participants’ needs, demonstrating the value of linking clinical and community efforts to support older adults.
