Effect of community health worker home visits on antenatal care and institutional delivery: an analysis of secondary outcomes from a cluster randomised trial in Mali

This study sought to report upon secondary outcomes of the Proactive Community Case Management cluster randomized trial conducted between December 2016 and April 2020 in Mali. This study evaluated whether proactive home visits can improve antenatal care (ANC) access at a population level compared with passive site-based care. CHWs in the intervention arm proactively visited all homes to provide care, while CHWs in the control arm provided the same services at fixed community health posts to care-seeking patients. Results demonstrated higher improvements in ANC access in the intervention arm compared to the control group, while both arms achieved large improvements in institutional delivery compared to baseline. Overall, the study’s findings suggest that home visits, when combined with user fee removal, professional CHWs, and expanded primary care, can offer an effective strategy for improving ANC access and delivery in settings such as Mali.
