This study developed a predictive model to identify underperforming community health workers (CHWs) in Uttar Pradesh, India, using data from over 140,000 CHWs. The model accurately pinpointed struggling CHWs and high-risk clusters early, enabling targeted support and resource allocation. This approach can improve CHW performance, motivation, and healthcare outcomes while advancing the use of data and technology in strengthening CHW programs.
