The goal of this paper was to outline a scalable digital health-enabled community-centered care model (D-CCC) aimed at empowering future CHWs using human-in-the-loop artificial intelligence. This D-CCC model was developed from a multidisciplinary collaboration among nursing, computer science, engineering, and human-computer interaction experts. It transforms manual and restricted aspects of CHW work into a scalable, digital, and intelligently automated space, aimed at increasing the quality of services delivered, in terms of personalization, cultural appropriateness, and timeliness. With this model, authors suggest there may be a path to expanding the community-centered health and social care ecosystem through improving digital connectedness while simultaneously empowering the CHW workforce.
