This study aimed to develop a simple screening model for detecting depression in elderly diabetic patients, suitable for use by community health workers. Using data from 309 patients, a predictive nomogram was created, based on factors like glucose monitoring, exercise habits, income, sleep disorders, and family support scores. The model demonstrated strong accuracy in both development and validation cohorts, with good calibration and clinical utility. This tool can effectively guide community health workers in identifying depression early among elderly diabetic patients.
