This study is testing whether adding simple nutrition counseling to a community health worker program can help low-income adults better manage high blood pressure. Participants receive guidance on reading food labels, eating healthy on a budget, and making practical food choices. The study will measure whether the approach is practical, well accepted, and whether it improves diet and blood pressure over four months. The results will help decide if this model should be expanded to support heart health and other diet-related conditions.
